Please fill out the forms below to provide your input on the Centre County 2025 Hazard Mitigation Plan! All forms below are linked to a digital form. If you would like a copy that you can download and print of any of the forms reach out to Devon Benne at Devon.Benne@mbakerintl.com.
Use this form to provide information on changes to the frequency of occurrence, magnitude of impact, or geographic extent of hazards identified in the 2021 Plan. In addition, the form asks for identification of any additional hazards that stakeholders believe should be considered in the 2025 Plan.
Use this form to identify the unique impacts of hazards in your municipality and provide any additional comments on hazard profiles or rankings. The hazards are listed in the order that they were ranked through countywide risk ratings for the 2021 Hazard Mitigation Plan Update.
This form asks you to look at your existing capabilities to implement mitigation projects. Use this worksheet from FEMA Region 3 for reference.
This form asks about your floodplain management and your status in the National Flood Insurance Program (NFIP). Use this worksheet from FEMA Region 3 for reference.
